Melasma (also known as chloasma or the mask of pregnancy when present in pregnant women is a tan or dark or brown skin discoloration. Although it can affect anyone, melasma is particularly common in women, especially pregnant women and those who are taking oral or patch contraceptives or hormone replacement therapy (HRT) medications.
CAUSES OF MELASMA
Melasma is thought to be the stimulation of melanocytes (cells in the dermal layer which transfer the melanin to the keratanocytes of the epidermis of skin that produce a pigmentcalled melanin) by the female sex hormones estrogen and progesterone to produce more melanin pigments when the skin is exposed to sun. Women with a light brown skin type who are living in regions with intense sun exposure are particularly susceptible to developing this condition.
Genetic predisposition is also a major factor in determining whether someone will develop melasma.
The incidence of melasma also increases in patients with thyroid disease.It is thought that the overproduction of melanocyte-stimulating hormone (MSH) brought on by stress can cause outbreaks of this condition. Other rare causes of melasma include allergic reaction to medications and cosmetics.
TREATMENT OF MELASMA
it may takes more than 6 months. pts may get varied results according to depth of pigment.
- Topical depigmenting agents, such as hydroquinone (HQ) either in over-the-counter (2%) or (4%) strength is a chemical that inhibits tyrosinase, an enzyme involved in the production of melanin gives quicker results but can not used for more than 3 months and stoping applying it melasma recures and prolong use causes aggravation of pigmentation and may be ochronosis.
Tretinoin,[6] an acid that increases skin cell (keratinocyte) turnover. This treatment cannot be used during pregnancy. triple combination preparation with HQ ,tretinoin and mometasone are also harmful in long term so avoided.
OCHRONOSIS AFTER LONG TERM USE OF HQ COMBINATION CREAM. - Azelaic acid (20%), thought to decrease the activity of melanocytes.[3]
- Tranexamic acid by mouth has shown to provide rapid and sustained lightening in melasma by decreasing melanogenesis in epidermal melanocytes
- Flutamide (1%),kojic acid,arbutin etc
- chemical peels
- Microdermabrasion to dermabrasion (light to deep)
- Galvanic or ultrasound facials with a combination of a topical crème/gel. Either in an aesthetician’s office or as a home massager unit.
- Laser but not IPL (IPL can make the melasma darker)
Oral medications and dietary supplements employed in the treatment of melasma include tranexamic acid, Polypodium leucotomos extract, beta‐carotenoid, melatonin, and procyanidin.[13]
In all of these treatments the effects are gradual and a strict avoidance of sunlight is required. The use of broad-spectrum sunscreens with physical blockers, such as titanium dioxide and zinc dioxide is preferred over that with only chemical blockers. This is because UV-A, UV-B and visible lights are all capable of stimulating pigment production.
Patients should avoid other precipitants including hormonal triggers.

અને BETNOVATE ,TENOVATE ,ELOCON ,PANDERM કે DERMI 5 જેવી ખરજવા કે એલર્જી ની સારવાર માં વપરાતી ટ્યુબો કિંમત માં ૧૫૦-૨૦૦ રૂપિયા ની અંદર મળે છે જે આ ડાઘ પર લગાવવાથી ડાઘ ઝડપ થી મટે છે .કેટલાક બ્યુટીપાર્લર માં આ દવાઓ ડબ્બી ઓ માં ભરી ને ઉંચી કિંમતે આપવામાં આવે છે.( ઉપર ની બધીજ દવાઓ થી સરેરાશ ૨-૩ મહિનામાં માં સારું લાગે) પણ

– MALA – D જેવી હોર્મોન્સ ની ગોળીઓ બંધ કરી બીજો વિકલ્પ અપનાવવો.
